What Is Harm OCD?
Harm OCD involves unwanted, intrusive thoughts, images, sensations, or urges centered on the possibility of causing harm to oneself or another person. These experiences can be extremely distressing. Someone may suddenly think, “What if I hurt someone I love?” They may then become frightened not only by the thought, but by what they believe having the thought might mean about them.
Intrusive Thoughts Are Not the Same as Intent
An intrusive thought does not automatically represent a person’s desires, intentions, character, or future behavior. People experiencing Harm OCD are often deeply disturbed by their intrusive thoughts. The problem can become worse when they attempt to achieve absolute certainty that they would never act on them.
Common Harm OCD Compulsions
Compulsions may include repeatedly checking whether someone was injured, avoiding knives or other triggering objects, avoiding loved ones, mentally reviewing past actions, testing emotional reactions,researching violent behavior, confessing intrusive thoughts, and repeatedly asking others for reassurance. Mental checking can be particularly difficult to recognize.
Why Does Harm OCD Continue?
OCD frequently demands certainty. The individual may believe, “I need to know with 100% certainty that I would never hurt anyone.” Because absolute certainty is difficult or impossible to achieve, the person continues checking. Each attempt to obtain certainty can reinforce the importance assigned to the intrusive thought.
Treatment for Harm OCD
ERP-based treatment can help individuals learn to experience uncertainty and intrusive thoughts without responding with compulsions. Treatment does not involve encouraging harmful behavior. A qualified clinician should also assess thoughts involving actual intent or plans to harm oneself or others because those concerns require appropriate evaluation and may differ clinically from OCD-related intrusive thoughts.
