What Is Symmetry OCD?
Symmetry OCD involves obsessive discomfort or anxiety related to things feeling uneven, incomplete, asymmetrical, imperfect, or simply “not right.” For some individuals, there may not be a specific feared catastrophe. Instead, the discomfort itself can become overwhelming. A person may feel unable to continue with an activity until something feels exactly right.
Common Symptoms
Symmetry and “just right” compulsions can include repeatedly arranging objects, aligning items, rewriting letters or words, repeating actions, touching objects a specific number of times, walking through a doorway again, restarting tasks, or performing movements equally on both sides of the body. Someone might touch something with their left hand and then feel compelled to touch it with their right hand to make the sensation equal.
Is Perfectionism the Same as OCD?
No. Someone can be highly organized, detail-oriented, or perfectionistic without having OCD. With OCD, the behavior is connected to obsessions, distress, compulsive urges, or a powerful need to achieve a particular feeling of completeness or correctness. The rituals may consume substantial amounts of time and interfere with school, work, relationships, or routine activities.
The “Just Right” Experience
Many people with this presentation describe difficulty explaining exactly what is wrong. They simply know: “It doesn’t feel right yet.” They may repeat the behavior until they experience a temporary sense of completeness. Unfortunately, responding to that sensation with another ritual can strengthen the cycle.
Treatment for Symmetry OCD
ERP treatment can involve gradually experiencing imperfection, asymmetry, or the “not right” sensation without correcting it through compulsions. Treatment progresses according to the individual’s symptoms and clinical needs. The goal is not to force someone to become disorganized. It is to increase the person’s freedom to decide whether something actually needs correcting rather than feeling controlled by a compulsive urge.
